Short-Term Rehabilitation. The goal of the rehab program at Evergreen Commons is to help residents achieve the skills they need so they can return home quickly and safely. We provide rehabilitation services for patients recovering from joint replacement surgery, a stroke or a prolonged hospital stay. We can also provide wound care, trach care, and IV therapy. Our physical, occupational and speech therapists are experienced in geriatric rehabilitation, carefully evaluate each resident and provide a program of individualized care. Family members are welcome to attend therapy sessions to make the transition home easier. We can also set up a home evaluation to ensure the home environment is safe and arrange for delivery of any necessary equipment or services.
