Admissions Form (pdf)

Admissions Procedure. Thank you for your interest in Evergreen Commons senior care facility. To get the process started, we will need the following information:

  • A completed Admissions Form (download Admissions Form here). The downloaded Admissions Form can be printed out and mailed to: Director of Admissions, Evergreen Commons, 1070 Luther Road, East Greenbush, NY 12061 or bring the completed form to the Admissions Office.
  • A medical report from the doctor or hospital of the person seeking admission.
